DAY IN THE LIFE
In-Home Visits: What to Expect
Our goal is to bring exceptional, personalized care to people, wherever they are.
We provide you with high-level insights about each patient so you walk in prepared. We do not pre-populate diagnoses. You are responsible for all clinical assessment and validation during the encounter.
You introduce yourself, confirm your credentials and take a moment to put the patient at ease. You’re not rushing to the first question. You’re establishing that this is a safe, respectful visit.
You get 45-60 minutes to have a meaningful interaction with each individual, checking vitals, discussing medications, providing preventive screenings and more. You have the time to review physical health, emotional well-being and the everyday needs – food, transportation, housing, connection – that affect how someone actually lives.
Documentation is completed with the same care as the visit itself. Our quality review process ensures accuracy and audit-readiness. This isn’t paperwork for its own sake, it’s the record that drives what happens next to support better patient care.
You flag what needs to take place next. You connect the member back to their primary care provider, specialist or community resource based on their unique needs. You make sure that the right people know what you found.
At Care Connectors, you’re part of a team. Clinical leaders are accessible to answer your questions. The organization provides you with ongoing training and support – and holds you accountable to the same high standards we expect of all of our team members.

Frequently Asked Questions
Schedules are flexible – built around your availability and the needs of your market. Most Care Connectors clinicians structure their own schedules within the framework of patient visit needs. Both full-time and part-time arrangements are available.
In-home visits last about 45 minutes to an hour per patient, excluding drive time. Our virtual visits take about 30 minutes. Our model is designed to give you ample time to have genuine conversations and provide whole-person care and personalized action plans.
You’ll have responsive clinical leadership, evidence-based guidelines and ongoing education. You’re never working in isolation. When you have a complex case or clinical question, there will always be someone you can reach.
You’ll complete thorough, structured documentation during and after each patient visit. We’ll equip you with the training, tools and support you need to be successful.
We’re actively expanding across the country. Submit your information, and we’ll let you know about openings in your area. We also have flexible virtual visit options available.
Our role structure varies, and we will discuss the specifics with you during our initial conversation. Full-time positions qualify for medical and other benefits.
