We know that having someone new come to your home may feel uncomfortable. So here’s a step–by-step view of what actually happens.
You’ll hear from us by phone, mail, email, text and, sometimes, directly through your health plan. We’ll set up a time that works for you. Caregivers and loved ones are welcome.
For in-home visits, a licensed clinician will arrive at the agreed time, show their ID and introduce themselves. There’s no equipment to set up, no forms to fill out before they arrive. Just a clinician ready to listen and support your needs.
This isn’t a quick check-up. We sit down and talk — about your health, your medications, how you’ve been feeling, and what’s been on your mind. We check your blood pressure and a few other things. We ask about your daily life and what’s making things harder or easier. We check your home environment for safety and fall risks.
At the end of the visit, we talk through what we found and what might help. We’re not here to tell you what to do — we’re here to help you figure out your next steps.
Care Connectors works alongside your regular doctor and care team — not instead of them. We share what we found so your doctor has the full picture. We can also help connect you to other services and schedule appointments if you need them.
