While the home is a primary modality for our care visits, we offer other flexible options – including virtual, mobile units, in-clinic and community events – to optimize access for members who need support the most.
Despite your investments in quality and care management programs, many individuals don’t respond to calls, delay care or struggle to navigate the system. Our licensed clinicians conduct comprehensive personal health and well-being visits – right in the member’s home or wherever they’re most comfortable. These visits supplement annual wellness check-ups and provide a fuller picture of the person’s health through extended one-on-one clinician time to:

We bring a proven model for driving improvements across more than a dozen of the most impactful HEDIS quality measures – from HbA1c testing and diabetic eye exams to breast and colorectal cancer screenings. Powered by our NCQA-certified HEDIS analytics or your own data, we conduct targeted outreach, deliver preventive care and screenings, and close the loop with other providers in the network. We take the time to understand member needs, build relationships and create the trust required for them to actively engage in their care and follow-through on next steps. Our flexible care delivery options – including in-home, virtual, in-clinic, at events, mobile units and labs in an envelope – help maximize gap closure.
Based on the trust our clinicians establish with members during the initial Personal Health Visit, we’re able to effectively re-connect with them to support additional needs called out in their personalized care plan. For example, individuals may need primary care or other referrals, medication adjustments or social support, like finding healthy food or transportation. We also can support transitions of care from an acute or sub-acute setting back to the home.
